Abstract: Kidney transplantation represents the treatment of choice for patients with end-stage chronic kidney disease because it improves survival and quality of life compared with renal replacement therapies. However, graft survival continues to be limited by immune mechanisms that promote rejection and the development of chronic injury. In this context, monocytes play a fundamental role in the innate immune response, as they participate in inflammation, antigen presentation, and T-lymphocyte activation. Likewise, the glycoprotein CD147 has been identified as a molecule involved in monocyte migration, activation, and recruitment, in addition to inducing the production of matrix metalloproteinases (MMP-2 and MMP-9), proteins associated with tissue remodeling and the progression of graft injury. The objective of the present study was to evaluate the effector response of monocytes obtained from kidney transplant recipients following stimulation with CD147 and its possible association with graft rejection. Specifically, the primary culture of monocytes obtained from peripheral blood was standardized, and the experimental conditions were optimized to analyze the production of MMP-2 and MMP-9 in cell culture supernatants. For this purpose, peripheral blood samples were obtained from kidney transplant recipients, and mononuclear cells were isolated by density-gradient centrifugation using Ficoll. Subsequently, monocytes were separated by plastic adherence and stimulated with different concentrations of CD147 for different incubation periods. In parallel, PBMC cultures were performed under similar conditions. The procedures for reagent preparation, cell culture, total RNA extraction and quantification, as well as the determination of MMP-2 and MMP-9 by ELISA assays, were standardized. The standardization of these methodologies provides a reliable experimental platform for future evaluations of the inflammatory response induced by CD147 in monocytes from transplant recipients. This study lays the groundwork for investigating the role of CD147 as a potential biomarker of immune activation and as a potential therapeutic target to improve the early detection of rejection and contribute to the development of strategies aimed at increasing renal graft survival.
El trasplante renal representa el tratamiento de elección para pacientes con enfermedad renal crónica terminal debido a que mejora la supervivencia y la calidad de vida en comparación con las terapias de reemplazo renal. Sin embargo, la supervivencia del injerto continúa limitada por mecanismos inmunológicos que favorecen el rechazo y el desarrollo de daño crónico. En este contexto, los monocitos desempeñan un papel fundamental en la respuesta inmune innata, ya que participan en la inflamación, la presentación de antígenos y la activación de linfocitos T. Asimismo, la glicoproteína CD147 ha sido identificada como una molécula involucrada en la migración, activación y reclutamiento de monocitos, además de inducir la producción de metaloproteinasas de matriz (MMP-2 y MMP-9), proteínas relacionadas con el remodelado tisular y la progresión del daño del injerto. El presente trabajo tuvo como objetivo evaluar la respuesta efectora de monocitos provenientes de pacientes con trasplante renal tras la estimulación con CD147 y su posible relación con el rechazo del injerto. De manera específica, se estandarizó el cultivo primario de monocitos obtenidos de sangre periférica y se optimizaron las condiciones experimentales para analizar la producción de MMP-2 y MMP-9 en los sobrenadantes celulares. Para ello, se obtuvieron muestras de sangre periférica de pacientes con trasplante renal y se aislaron células mononucleares mediante gradiente de densidad con Ficoll. Posteriormente, los monocitos fueron separados por adherencia plástica y estimulados con diferentes concentraciones de CD147 durante distintos tiempos de incubación. Paralelamente, se realizaron cultivos de PBMC bajo condiciones similares. Se estandarizaron los procedimientos de preparación de reactivos, cultivo celular, extracción y cuantificación de ARN total, así como la determinación de MMP-2 y MMP-9 mediante ensayos ELISA. La estandarización de estas metodologías proporciona una plataforma experimental confiable para futuras evaluaciones de la respuesta inflamatoria inducida por CD147 en monocitos de pacientes trasplantados. Este estudio sienta las bases para investigar el papel de CD147 como posible biomarcador de activación inmunológica y como potencial blanco terapéutico para mejorar la detección temprana del rechazo y contribuir al desarrollo de estrategias dirigidas a incrementar la supervivencia del injerto renal.
