Title: Fortalecimiento de la seguridad del paciente mediante la intervención del servicio de farmacia hospitalaria y la identificación de factores predisponentes para la prevención de reacciones adversas a medicamentos

Abstract: This final social service report documents a research and care intervention project carried out by Emanuel Moguel Díaz at the General Regional Hospital (HGR) No. 1 Dr. Carlos Mac Gregor Sánchez Navarro of the IMSS, from June 3 to December 3, 2025. The central objective was to strengthen hospitalized patient safety in high-specialty environments, transitioning from a traditional and passive pharmacy model to an active and intentional pharmacovigilance approach. The implemented methodology was based on Pharmacotherapeutic Follow-up (SFT) using the structured SOAP (Subjective, Objective, Assessment, Plan) method, prioritizing the polypharmacy population in the fifth-floor hospitalization area. During a prospective three-month data collection period, and adhering to NOM-220-SSA1-2016, a total of 45 safety notifications were captured and analyzed. The results obtained revealed that Medication Errors (ME) and Adverse Drug Reactions (ADR) constituted the main challenges, with each category representing 40% of the total notifications. In terms of severity, 55.6% of the events were mild, 33.3% were moderate, and 11.1% were severe. Among the most significant clinical findings was the critical detection of a systemic trend towards extreme sub-therapeutic concentrations of Tacrolimus in transplant patients, a vital interception that prevented the imminent risk of organ rejection. Additionally, polypharmacy was identified as the main predisposing factor for the development of ADRs and opportunistic infections associated with the use of carbapenems. The project also managed to resolve a severe and prevalent pharmacodynamic interaction between amlodipine and calcium supplements that caused refractory hypertension in nephrology patients. Collectively, the timely pharmaceutical interventions achieved a high acceptance rate of 71.1% by the multidisciplinary medical team. The report concludes by empirically demonstrating that the standardization of active pharmacovigilance, led by the clinical pharmacist using tools such as the SOAP model, constitutes an indispensable safety barrier to optimize treatments, prevent risks, and guarantee the comprehensive protection of the hospitalized patient.
Este informe final de servicio social documenta un proyecto de investigación e intervención asistencial realizado por Emanuel Moguel Díaz en el Hospital General Regional (HGR) No. 1 Dr. Carlos Mac Gregor Sánchez Navarro del IMSS, durante el periodo del 3 de junio al 3 de diciembre de 2025. El objetivo central fue fortalecer la seguridad del paciente hospitalizado en entornos de alta especialidad, transitando de un modelo de farmacia tradicional y pasiva a un enfoque de farmacovigilancia activa e intencionada. La metodología implementada se basó en el Seguimiento Farmacoterapéutico (SFT) utilizando el método estructurado SOAP (Subjetivo, Objetivo, Análisis, Plan), priorizando a la población con polifarmacia en el área de hospitalización del quinto piso. Durante un periodo prospectivo de tres meses de recolección de datos, y rigiéndose por la NOM-220-SSA1-2016, se captaron y analizaron un total de 45 notificaciones de seguridad. Los resultados obtenidos revelaron que los Errores de Medicación (EM) y las Reacciones Adversas a Medicamentos (RAM) constituyeron los retos principales, representando cada categoría el 40% de las notificaciones totales. En términos de gravedad, el 55.6% de los eventos fueron leves, el 33.3% moderados y el 11.1% graves. Entre los hallazgos clínicos más significativos destaca la detección crítica de una tendencia sistémica hacia concentraciones extremas subterapéuticas de Tacrolimus en pacientes trasplantados, intercepción vital que previno el riesgo inminente de rechazo de órganos. Además, se identificó a la polifarmacia como el principal factor predisponente para el desarrollo de RAM e infecciones oportunistas asociadas al uso de carbapenémicos. El proyecto también logró resolver una interacción farmacodinámica severa y prevalente entre amlodipino y suplementos de calcio que provocaba hipertensión refractaria en pacientes nefrópatas. En conjunto, las intervenciones farmacéuticas oportunas lograron una alta tasa de aceptación del 71.1% por parte del equipo médico multidisciplinario. El informe concluye demostrando empíricamente que la estandarización de la farmacovigilancia activa, liderada por el farmacéutico clínico mediante herramientas como el modelo SOAP, constituye una barrera de seguridad indispensable para optimizar los tratamientos, prevenir riesgos y garantizar la protección integral del paciente hospitalizado.
