Nivel de Atención en el Manejo Estomatológico

Abstract

A cross-sectional and prospective investigation was carried out at the “Dr. Ignacio Chávez” of the ISSSTE from February 2023 to January 2024 to beneficiaries aged 6 or older of both sexes, on the type of dental care by levels of care. The first level care activities were: hygiene and prophylaxis, for the secondary level: early diagnosis, temporary fillings, scaling, permanent fillings with resin, curettage and extractions and the tertiary level corresponded to referrals to specialties. The information analysis was obtained with measures of central tendency and cross tables. (Excel and SPSS) More than 50% of patients over 45 years of age received first level of care, for the second level the same proportion for older adults and between 10% and 22% for the third level. When analyzing the activities of two pathologies with the highest prevalence according to the WHO1 (periodontopathies and edentulism), curettage was used by 82% of both sexes and extractions were affected by 18% more frequently in men. Regarding this last point, when comparing with activities carried out in the Coyoacán Mayors Office in 2021,2 the execution of extractions was 28%, higher than the present work. It is important to increase the level of primer from the first years of life, as well as reinforce the importance of caring for the dental support tissues to avoid both the development of periodontopathies and tooth loss.

Se realizó una investigación transversal y prospectiva en la Clínica Familiar “Dr. Ignacio Chávez” del ISSSTE de febrero de 2023 a enero de 2024 a derechohabientes de 6 ó más años de edad de ambos sexos, sobre el tipo de atención estomatológica por niveles de atención. Las actividades de primer nivel de atención fueron: higiene y profilaxis, para el secundario: diagnóstico precoz, obturaciones temporales, detartraje, obturaciones permanentes con resina, curetaje y extracciones y el nivel terciario correspondió a referencias a especialidades. El análisis de la información se obtuvo con medidas de tendencia central y tablas cruzadas. (Excell y SPSS) Más del 50% de los pacientes mayores de 45 años recibieron primer nivel de atención, para el segundo nivel la misma proporción para adultos mayores y entre el 10% y 22% para el tercer nivel. Al analizar las actividades de dos patologías de mayor prevalencia según la OMS1 (periodontopatías y edentulismo), la realización de curetaje ocupó el 82% afectando igual en ambos sexos y las extracciones el 18% con mayor frecuencia para los hombres. Sobre este último punto, al comparar con actividades realizadas en la Alcaldía Coyoacán en 2021,2 la realización de extracciones fue del 28%, superior al presente trabajos. Es importante incrementar el primer nivel desde los primeros años de vida, así como reforzar la importancia del cuidado de los tejidos de soporte dentario para evitar tanto el desarrollo de periodontopatías como la pérdida dental.
