Ética de la salud colectiva

Abstract

From the perspective of Social Medicine and Collective Health, the ethic searches to make available a reflection about what ethics principles are possible, and obligatory, to apply in the health field, considering not only the medical proceedings or the clinical investigation, but the impact of health public policies on the different spaces of society. In order to contribute with this task, this work looks for to identify and to delimit the ethical principles that allow the construction of an ethics of the Collective Health. The way we followed for such aim was, in first term, to locate what science understands as health in general and where the limits are of Collective Health. This was carried out making an historical and epistemological revision on what health is; a documentary revision of the theoretical contributions of the field of collective health, and a hermeneutical analysis on the concept of Collective Health. For the formulation of the ethical principles that could or have be applied initially to the SC it carries out a count of the main ethical schools; immediately we continue with the hermeneutical critic of Adela Cortina to identify the principles that could be useful in this field of knowledges and, finally, appear the principles proposed for the SC, which they are, that is to say: fairness, distributive justice, subsidiariary, caution, participation, tolerance and reciprocity.

Desde la perspectiva de la Medicina Social (MS) y de la Salud Colectiva (SC) la ética busca ser una reflexión cimentada en una serie de principios capaces de considerar no el acto médico en el servicio asistencial o la investigación clínica, sino el impacto de las acciones sanitarias y las políticas públicas de salud sobre los distintos ámbitos de la sociedad. Para contribuir con esta tarea, el propósito de este trabajo es identificar y delimitar los principios éticos que permitan la construcción de una ética de la SC. El camino que se siguió para tal fin fue, en primer lugar, ubicar qué es lo que se comprende por salud en general y dónde se encuentran los límites del campo de saberes conocido como Salud Colectiva. Esto se llevó a cabo realizando una revisión histórica y epistemológica sobre lo que es la salud; una revisión documental de las contribuciones teóricas de dicho campo, y un análisis hermenéutico interpretativo sobre el concepto de Salud Colectiva. Para la formulación de los principios éticos que podrían o deberían aplicarse a la SC inicialmente se lleva a cabo un sucinto recuento de las principales escuelas éticas; enseguida se recurre a la hermenéutica crítica de Adela Cortina para identificar los principios que podrían ser útiles en este campo de saberes y, finalmente, se presentan los principios propuestos para la SC, que son, a saber: equidad, justicia distributiva, solidaridad, responsabilidad, subsidiariedad, precaución, participación, tolerancia y reciprocidad.
