Derecho a la salud de los pueblos indígenas : Estudio crítico de las políticas y prácticas de la salud intercultural en el Hospital Comunitario de Tonalapan, Veracruz.

Abstract

The purpose of this research was the identification of the ways in which the policies of intercultural health, their implementation and the practices of interculturality, in the Community Hospital of Tonalapan, promote or hinder the realization of the right to health for indigenous peoples. The present paper has a qualitaitive research and interpretivist approach,from a historical-hermeneutical; relational; and critical point of view To this end this study has been divided into three phases: macro,meso and micro, each one of them relating to the international and regional levels (the Americas); federal and state (México and Veracruz); and, local (Tonalapan Hospital and Huazuntlán). Two types of strategies were utilized: documentary research and case study. The screening consisted. First, in a thorough review and understanding of international and regional treaties on the right to health, in general, and on the right to health of indigenous peoples, especially. In a second phase, we reviewed the laws, regulations, plans, and programs, federal and the state of Veracruz, concerning intercultural health. And, finally interviews were conducted to the health staff of the Hospital of Tonalapan and traditional birth attendants and women of Huazuntlán, who were treated during pregnancy and childbirth, both by traditional midwife as in hospital. The information obtained in each moment is analyzed and interpreted taking into account the categories of interculturality, discrimination, transactional processes and hybridization. Among the results I can mention the presence of discrimination in the policies and the practices of intercultural health, as well as disregard for the concept of interculturality and inappropriate practices relating to health with an intercultural approach.

El propósito de esta investigación fue la identificación de las formas en que las políticas de salud intercultural, su implementación y las prácticas de interculturalidad, en el Hospital Comunitario de Tonalapan, promueven u obstaculizan la realización del derecho a la salud de los pueblos indígenas. El presente trabajo es una investigación cualitativa interpretativista, desde un enfoque histórico-hermenéutico; relacional; y un posicionamiento crítico. Para tal fin este estudio se dividió en tres fases: macro, meso y micro, cada una de ellas referentes a los ámbitos internacional y regional (Las Américas); federal y estatal (México y Veracruz); y, local (Hospital de Tonalapan y Huazuntlán). Se utilizaron dos tipos de estrategias: la investigación documental y el estudio de caso. La pesquisa consitió, primeramente, en una revisión minuciosa y comprensión de los tratados internacionales y regionales sobre el derecho a la salud, en general, y sobre el derecho a la salud de los pueblos indígenas, especialmente. En un segundo momento, se revisaron las leyes, reglamentos, planes y programas, federales y del estado de Vearcruz, concernientes a la salud intercultural. Y, por último, se llevaron a cabo entrevistas al personal de salud del Hospital de Tonalapan y a parteras tradicionales y mujeres, de Huazuntlán, que fueron atendidas durante el embarazo y parto, tanto por partera tradicional como en hospital. La información obtenida en cada momento se analizó e interpretó tomando en consideración las categorías de interculturalidad, discriminación, procesos transaccionales y de hibridación. Entre los resultados puedo mencionar la presencia de discriminación en las políticas y las prácticas de la salud intercultural, así como desconocimiento del concepto de interculturalidad y prácticas inadecuadas referentes a la salud con enfoque intercultural.
