Atención Odontológica en Paciente con Hipertensión Arterial y Evento Vascular Cerebral con Tratamiento Antiagregante Plaquetario. Reporte de caso

Abstract

The Social Service was provided in the Ministry of Health of Mexico City (SEDESA), in the Health Centers and Specialty Clinics of the Cuauhtémoc Health Jurisdiction in the Salud en tu casa program in the period from February 2021 to January 2022. During this period, home visits were made to people not entitled to social security with conditions such as disability, prostration, pregnancy, advanced age and/or terminal illness. The dental care provided to the patients of the program involved, in the first instance, the completion of the Clinical History and later progress notes (PSOAP format). The information collected in these documents was requested by the program staff prior to carrying out any intervention and depending on the patients state of health and consciousness, it was requested from the patient or from a family member and/or responsible caregiver, who authorized the care that was provided within of the home, which implies a great commitment and responsibility, since they opened the doors of their home to us in order to provide them with the best possible care. In general, the type of treatments that were performed on the patients of the program were odontoxesis with the use only of masks, simple extractions, relining of prostheses with tissue conditioner and educational talks, due to the epidemiological situation due to the spread of the SARS-COV virus. 2, therefore, it was not always possible to carry out home visits, and other administrative activities were carried out such as telephone follow-up of patients, logistics for anti-COVID-19 vaccination, in addition to educational talks in health centers. health, Diploma in Public Health and Comprehensive Medicine via remote, health conferences, presentations of educational material (flip charts) and in support of the staff of the Health Jurisdiction, collection of nasal samples for detection of COVID-19. In particular, for this work, a clinical case report was made regarding first-level dental care in a patient with Systemic Arterial Hypertension and consequences of a Cerebral Vascular Event (CVE) under antiplatelet treatment. Both for the patient in this case, and for patients in general treated at home, the greatest benefit obtained was the monitoring of their health, and that the fact of having mobility limitations did not prevent them from receiving care, and thus prevent Their health deteriorated, since they also worked together with the patients caregivers.

El Servicio Social fue prestado en la Secretaría de Salud de la Ciudad de México (SEDESA), en los Centros de Salud y Clínicas de Especialidades de la Jurisdicción Sanitaria Cuauhtémoc en el programa Salud en tu casa en el periodo comprendido de febrero de 2021 a enero de 2022. Durante dicho periodo se realizaron visitas domiciliarias a personas no derechohabientes de seguridad social con condiciones como discapacidad, postración, embarazo, edad avanzada y/o enfermedad terminal. La atención odontológica que se prestó a los pacientes del programa implicó en primera instancia la realización de la Historia Clínica y posteriormente notas de evolución (formato PSOAP). La información recabada en estos documentos fue solicitada por el personal del programa previo a realizar cualquier intervención y dependiendo del estado de salud y de conciencia del paciente se solicitaron a este o a algún familiar y/o cuidador responsable, quien autorizó la atención que se brindó dentro del domicilio, lo cual implica un gran compromiso y responsabilidad, ya que nos abrieron las puertas de su casa para poder brindarles la mejor atención posible. En general, el tipo de tratamientos que se realizaron a los pacientes del programa fueron odontoxesis con uso únicamente de caretas, extracciones simples, rebases de prótesis con acondicionador de tejidos y pláticas educativas, debido a la situación epidemiológica por propagación del virus SARS-COV-2, por lo cual, no siempre fue posible realizar las visitas domiciliarias, y se realizaron otras actividades de tipo administrativo como seguimiento telefónico a los pacientes, logística para vacunación anti COVID-19, además de pláticas educativas en centros de salud, Diplomado de Salud Pública y Medicina Integral vía remota, jornadas de salud, presentaciones de material educativo (rotafolios) y en apoyo al personal de la Jurisdicción sanitaria, toma de muestras nasales para detección de COVID-19. En particular, para el presente trabajo, se realizó el reporte del caso clínico referente a la atención odontológica de primer nivel en paciente con Hipertensión Arterial Sistémica y secuelas de Evento Vascular Cerebral (EVC) bajo tratamiento antiagregante plaquetario. Tanto para la paciente de este caso, como para los pacientes en general tratados en sus domicilios, el mayor beneficio obtenido fue el seguimiento a su salud, y que el hecho de tener limitaciones de movilidad no impidiera que recibieran la atención, y así evitar que su salud se deteriorada, ya que además, se trabajaba en conjunto con los cuidadores de los pacientes.
